Meeting Change Request Form

Please fill out this form as thoroughly as possible. The request will be sent to multiple recipients for review. Our goal is to complete your changes within 72 hours. Per policy, we list only groups based in Santa Cruz County, California. For meetings at the Ben Lomond Fellowship, contact your media coordinator to update the Ben Lomond Fellowship site. Those changes will be copied to the Santa Cruz County schedule.

Include meeting description or notes to attendees (i.e. In the back room or Chip meeting on 3rd Wednesday), if applicable. Can be used for special instructions to the meeting change team.
